Those applying for a place on the basis of Christian faith are required to submit both Supplementary forms A & B.
If applying on a faith basis, as well as completing form A yourself, please ask your pastor, minister or spiritual leader to complete Supplementary Information Form B (for church leader) before the 16th May.
It is entirely the responsibility of the applicant to ensure that both forms A & B are supplied to the admissions authority.
